Our customizable trade program forms are the starting point for building your b2b wholesale or trade programs on Shopify. You can easily gather the qualifying information you need from customers using these forms.
Create a trade form in the application
On the left hand side in the Shopify admin, select Sign-Up Forms from the application menu. From here you can create your first sign-up form and configure the questions, question type, if the field is required & adjust the form layout using columns for sizing or drag-and-drop to reorder questions.
When you're done, go ahead and hit save. You can always make changes to the form later.
Be sure to note down the form ID that's generated after creating your form. You'll need this in the next step.
Adding the b2b sign-up form to a page in Customize Theme
Noting the form ID in the Sign-Up Form table, go ahead and move over to Online Store -> Customize Theme.
Here we're going to add the B2B Sign Up Form application block to a page. It can be added to any page.
We typically recommend creating a new page template to add the app block to, this way you can use other native theme blocks to customize the page however you'd like. If you do this, make sure to then create a page that leverages the new page template.

Finally, input the form ID - you should see the loading animation go away replaced with the form you previously defined in the application admin. Go ahead and customize the app block settings to match your store.

With your new form page setup, you're ready to start collecting sign-ups for your b2b wholesale trade program. As applications are submitted, they'll start appearing in your B2B Wholesale & Trade Discount application dashboard.
